A collograph print is a collage.  Any number of materials can be used in a collograph print.  I tend to use materials from nature such as leaves, grasses, flowers or anything else that suits my fancy (weeds are pretty interesting) or templates that I makes.  In this particular collograph, I made a bonsai tree template.  There are several ways to do a collograph; I generally ink the plate with the background color and design and transfer it to the paper either by hand pressing or running it through the press.  Once the background is dry, I ink the materials or template I'm using, lay it on the background and then transfer by hand pressing or running through the press.
